What Happens to Your Jawbone After a Tooth Extraction?

When a tooth is removed, the jawbone no longer receives the stimulation it needs to maintain its density and volume. Over time, the bone begins to resorb (shrink) in a process called bone resorption. This happens because the chewing forces that once traveled through the tooth root are no longer present.

Bone loss is most rapid in the first three to six months after extraction. Studies show that 29-63% of horizontal bone loss and 11-22% of vertical bone loss can occur during the first six months if no preservation techniques are used. This means the longer you wait, the more bone you may lose.

Even with some bone loss, dental implants in Chapel Hill can still be an option. It just takes a little more planning, and occasionally a bone graft, to create a solid foundation for the implant.

Can You Get an Implant Years After Extraction?

What If You’ve Lost Bone?

Bone loss is a common concern for patients who have waited years to replace a missing tooth. But that doesn’t disqualify you from implants.

Bone grafting is a routine procedure that can rebuild the jawbone to support an implant. Your dentist may use bone from your own body, a donor, or a synthetic material to stimulate new bone growth. After the graft heals (typically 4-6 months), the implant can be placed.

In some cases, advanced techniques like alveolar ridge splitting can expand the bone from within, making implants possible even with existing bone loss.

Does Delayed Placement Affect Implant Success?

Research shows that both immediate and delayed implant placement have excellent long-term survival rates.

A large-scale retrospective study published in 2025 reported 98.4% survival for immediate implants and 98.6% for delayed implants, with no statistically significant difference between the two approaches. Another narrative review reported survival rates of 95–100% for immediate placement and 98–100% for delayed placement.

This means that even if you wait, you can still expect a highly predictable, long-lasting result.
